Please register by filling in one of the form options below.

Registration & Consent forms
This option can be filled in in Microsoft Word and emailed by following the steps below.
1.  Right click on the Registration & Consent forms link above these steps and select "Open in a new window"
     With the form page on your screen, look to the right upper tool bar and click on the down arrow to the right of
"Page".
2.  Click on "Edit with Microsoft Word"
3.  The form will open in MS Word. Depending on your connection, the form may take a few seconds to open.
     Click on the blank lines or around the blank lines. Your cursor should begin blinking on the blank line.
     You are ready to type your information.
4.  Select "File" at the top left corner when you are ready to email.
5.  Select "Send to" or "Send"
6.  Select "Mail Recipient" or "Email"
7.  Type in fellowshipblc@att.net in the "To:" space in your email program
8.  Click "Send"
